Dear Parents and Guardians,

The KFP doctor visited the school this morning to administer the nasal flu vaccine. However, we do not have a completed consent form for your child.

If you would like your child to receive the vaccine, please send the completed consent form into school with your child or email it to Brannoxtowncns@kwetb.ie by tomorrow, Wednesday 21st October.

We hope that the doctors will be able to return to the school at a later date to administer the vaccine to any children whose completed consent forms have been received.

I have attached a copy of the Nasal Flu Vaccination Consent Form for your reference.
